New Patient Forms
We get it. Life keeps you busy. The last thing you want is to come into our office and have to fill out reams of paperwork. Our new patient forms are 100% digital, so you can fill them out and submit them online at a time that is convenient for you.
Our digital forms are also HIPAA compliant, meaning that all of your information is encrypted and transmitted securely to protect your privacy. Of course, if you prefer paper forms, you can still download and print a PDF version and bring it with you to your first visit.
Patients who are over the age of 18 need complete the Adult New Patient Form. For children and teens under the age of 18, a parent or guardian needs to complete the Child & Teen New Patient Form.

The cost of orthodontic treatment depends on the severity of the issue(s) to be corrected. You will be able to discuss fees and payment options during your first visit with us.
We accept many insurance plans as well as several cost savings plans. Our office will contact your insurance company to determine your orthodontic benefits and will file all of the necessary paperwork on your behalf.
